The relationships between the Mongolian fold and the double eyelid
The Mongolian fold makes the distance between the eyes seem wider than the actual distance,and lets them form sharp or strait eyes. More importantly, a double eyelid surgery without correcting the fold can cause an undesirable line, and even cause it to become loose or faded.
When women have a consultation about the double eyelid surgery, most of them ask for clearer and wider eyes such as the look of an actress. However, a double eyelid surgery without epicanthoplasty will likely make the eyes look undesirable because this will result in a wide intercanthal(or interepicanthal) distance and short eye length.
So when one has a severe Mongolian fold or wider intercanthal distance than the horizontal length of an eye, double eyelids must be made by combining two of these surgeries. But, one can select only a double eyelid surgery if he or she wants, and it is also possible to undergo Magic epicanthoplasty solely in any state.
Mongolian fold is the most significant factor for determining the shape of the double eyelid line, but it is not well-known because massive scar formation and unsatisfied result may appear due to other previous methods.
It is now unnecessary to hesitate to have an epicanthoplasty because of the advent of Magic epicanthoplasty. Recently taking double eyelid surgery combined with Magic epicanthoplasty has become a new paradigm of plastic eyelid surgery.
Example) Example of an eye with a natural double eyelid and a slight Mongolian fold. The caruncle(red part) is exposed almost completely and the natural in-out folder is expressed. The eyeline is exposed fully up to the epicanthal angle(corner) and the ancillary eyelashes decorate eyes so beautifully.
Example) This shows a completion of a double eyelid surgery without the removal of the Mongolian fold. It shows the drooping of the upper lid skin.(above) It is a result of the buried suture method only, despite the fact that it was supposed to be done by Magic epicanthoplasty with excisional double lid surgery.
This in-folder portrays a stuffy image because the line cannot be connected fully to the canthal area, and the inner eyeline is almost buried by the skin above.
These kinds of double eyelids are very likely to get loose or fade away in a few months or years .

As you can see, the left eye is more severe than the right. The right eye (actually on left from the point of view of the reader) looks unnatural because the out-folder is designed above the Mongolian fold. The line will become unclear starting from the corner of the eye.
It is likely to fade away gradually. The left eye shows an in-folder type under the Mongolian fold. When there is Mongolian fold with strong tension, a double eyelid will look like the left eye where the line is lower and closer to the lashes.
Example) Combination of Magic epicanthoplasty and excisional method in the case of severe Mongolian folds. In-out folders are created, and the eyes have been changed into clear and vivid appearance by correcting Mongolian folds, which makes the eyes have a fierce look.
Examples) Sausage type of double eyelids were made when not taking epicanthoplasty before in severe type of Mongolian folds. Magic epicanthoplasty and revisional double eyelid surgery refined the images of eye in manner of natural and distinctive looks.
Example) This case shows the negative effect of Mogolian folds in the eyes with out-folder double eyelids. There is no continuation of line up to the medial corner of the eye(above). These types of double eyelid lines are usually located at a lower position near the epicanthal region. When the Magic epicanthoplasty is performed, it is frequently necessary to correct the double eyelid lines at the same time in the epicanthal area to obtain clear double eyelids.
Example) The effect of Magic epicanthoplasty to the double eyelid line. This is only when the Magic epicanthoplasty is performed without correcting the line. A stuffy and closed-eye appearance still remains after the surgery. However, some just prefer in-folder lines.
Example) Buried suture method (non-incisional double eyelid surgery) with Magic epicanthoplasty was performed to correct the lines. It shows slight out-folder lines after surgery.
Examples) These are the cases which show tension of Mogolian folds in the eyes of out-folder double eyelid. Clear lines were made by the combination of Magic epicanthoplasty and medial correction of double eyelid.
